Group Health Insurance Program subscribers and spouses can earn a $150 prepaid card for completing a quick health assessment, health check, and well-being activity by October 17, 2025. If you complete your health assessment by May 30, 2025, you’ll also be entered into a prize drawing. Get started at the Well Wisconsin online portal or download the Wellness at Your Side app.
